For 1.48, metadata on device not updated after upload?

Still on 1.48 due to xp: after transferring books (azw3) to voyage, the kindle does not show the changes. The job log does show:
'Upload books to device' and then 'Send metadata to device'.

Only when I manually run 'Update Cached Metadata on Device' does the kindle finally show the changes. Is this a bug with 1.48?

Update cached metadata on device only affects the calibre "Device" view. If you have used the Match Books option (Device view, right-click menu) then Update cached metadata will rebuild the metadata.calibre file on the device.

As soon as you use Send-to-device the AZW3 file should be written to the Kindle drive.
Once ejected, the Kindle will find all ebook files on the drive, unless it is malfunctioning. But it may take a bit of time to index it, specifically if there are many new books and/or the indexer jams .

Do I need to use the 'match book' option on each book I want to update or does doing one book then make 'update cached metadata' affect all books on the device?

I've also noticed that the covers on the device are smaller than expected, which is due to 1.48 not having a voyage profile. I tried your xp backport of calibre (thanks!) and now newly transferred books have the proper cover size, but is there a way to refresh the covers for existing books on the device?

You're welcome for the backports.

calibre does not have a way to refresh the books -- you'll need to resend them (or mess with custom scripts to extract the covers on-device and blah, which is work you probably don't want to do yourself, unless you happen to know python and have time on your hands, and only saves you a bit of time anyway).

And match books is only to make metadata in the Device View match the calibre library. If you needed to use it, you'd know already.
As I said, the only thing Match books does is update the single file "metadata.calibre" on the device.

When calibre sees an ereader for the first time, it looks for the books on the device and tries to match them with the books in the library. It does a pretty good job of this, but sometimes it doesn't match them correctly. The "match book" can be used to fix this. But, you then need to run "update cached metadata" to make this permanent. This updates the "metadata.calibre" file on the device.

After this, when you send a book to the device, calibre will update the "metadata.calibre" file on the device adding that book. You shouldn't need to do the match and cache update again unless you add books to the device using some other method. Or something happens to the "metadata.calibre" file.

To refresh the covers, I had no choice but to deleted the books from the device and re-uploaded them.

I also managed to fix the metadata issue but it was more luck than skill. Premise is that I have the latest calibre running on a win7 pc. Then I overwrite-copy one of the libraries+metadata.db to the winxp pc (kid's pc). I have another voyage, which I use with the win7 pc, and tried it with the winxp pc and it had no issues with the metadata after an upload. So on a whim, I connect the troubled voyage to the win7 pc, did an upload, and it updated fine. Then when I connect the same voyage back to the winxp pc, the uploads now are working fine. Weird.

Many devices that manage their own 'book shelf' view load that part only once, when they discover a NEW book.

To force a new view to load, you must make the existing entry go away.
The unfortunate side effect is any bookmarks/annotations can also go
