I took a look at that book and saw that it contains a badly coded advertising page created by the publisher. This is being converted poorly by Amazon, resulting in inconsistent formatting in the KFX file.

KFX input conversion plugin - Not working after Calibre 4.5
Bug description ??? THIS IS MY FIRST EVER POST - SO NOT SURE IF IT IS IN THE RIGHT PLACE.
I am using Mac Kindle 1.23.1 to download amazon purchased books. I am using KFX input conversion plugin 1.26.0 in Calibre. I keep up with the latest Calibre updates and was using version 4.7. I drag the .azw file from the "My Kindle Contents" folder to Calibre to import. The format in Calibre ENDS UP BEING "KFX-Zip" AND WHEN CONVERTING KFX-ZIP TO EPUB GET A 'CANNOT CONVERT BECAUSE OF DRM'. From my investigations into the matter found out that this may be a result of incorrect DRM removal on adding .azw file to Calibre and the KFX input conversion plugin (I think). I tried a whole raft of things to resolve to no avail, including zipping the Kindle files and changing the extension to KFX-zip. HOWEVER, I DID FIND A SOLUTION (A WORK AROUND). MY RESOLUTION:- I REINSTALLED CALIBRE VERSION 4.5. This is the last version of Calibre that I used successfully to upload .azw to Calibre resulting in KFX format (It is still using the 1.26.0 plugin) - IT WORKED. SO MY CONCLUSION IS THAT, FOR SOME REASON, BETWEEN VERSION 4.5 AND 4.7 THE "KFX INPUT CONVERSION PLUGIN" IS NOT WORKING AND .azw files ARE ENDING UP AS KFX-ZIP FORMAT IN CALIBRE OR I HAVE SLIPPED UP SOMEWHERE. |

I can confirm the bug report above. Reverting to calibre 4.5 solved the problem while still using latest versions of all plugins. Debug logs:
