Should I always edit books via the manager?
If you edit a PDF directly and it changes size, Calibre doesn't detect. Does the same happen if you open in-library epub directly in ebook-editor instead of pressing 'edit' button?

Yup, otherwise calibre would have to be constantly scanning files to check when they are changed by unrelated processes.
