On-page footnotes missing
Even though I enabled on-page footnotes in KOReader, they are not showing up by default, only when clicking the footnote number. I checked one ebook and the original is showing footnotes by default. The difference seems to be how Calibre handles footnotes, comparison below.

Is there a way to change conversion settings so that footnotes are untouched?

There's no difference in the code apart from changed class names which have no bearing on footnotes. No there is no way to prevent class names from being changed during conversion. If you want footnotes to work robustly you should use the dedicated footnote markup for them. (epub:type or role)
