Creating a TOC causes errors

I have a publisher that is consistently problematic with TOC in their ebooks. Usually there is just one entry, which is the beginning of the book. And then, there is usually a TOC in the book itself, which is just hyperlinked text and not an actual toc file. I would prefer a TOC that gets recognised by my ereader for the ease of use. I have tried generating one via edit ebook option, but if I do that, then my ebook gets manifest items missing errors and I'm not sure how to fix it.

Also another question: is there any options similar to file naming templates I could use for auto generating chapter names?

??? You want a tool to make up Chapter names??

I assume you really want a NCX or Nav TOC

Calibre has a TOC tool (page with hand icon) available from the Library (I usually use that to just remove odd entries, but it is the full deal) and the Editor. look at what that can do.
h# headers is a common auto detect trigger (remember you can style Headers if you do not want Bold or the size) I usually do this (your problem) task in the editor, because the chapters wer not coded for auto detect. The easy , simple ones are a S&R or use the editing toolbag (PI): Modify

well a direct translation of 'chapter -number-' option that I have been able to find doesn't work in my native language so I was hoping it was possible to customise that template a little bit if that makes sense. but I'm happy just using numbers.

I've been using that tool and it works in most cases except a select few(the manifest items missing error) and I'm trying to understand why so I can avoid it!
