Why does this book crash my Kobo Forma?

This side-loaded book makes my Kobo Forma crash as soon as I get to the end of the index at the beginning and try to go to the next page. Jumping directly to the first chapter using the TOC works, but the book crashes a few pages later anyway.

The non-scrambled book is only 2.3MB and epubcheck couldn't find faults with it; this crashes the Kobo both in EPUB and KEPUB formats.

Does anyone have any tips?

What happens if You convert from epub to epub with calibre? It could be be a dirty quick solution.

Unfortunately, it still crashes.

I took a quick look at the book and it didn't crash my Sage. On the other hand, it did crash my Forma. I also tried using kepubify to generate a kepub and that did not crash on either The only change I've made between the Sage and the Forma is the Sage has no patches installed while the Forma has the kobopatch RMSDK disable widows/orphans avoidance patch enabled. I tried disabling that item and the crash no longer occurred for me.. I've attached the two files I used to this message (I cleaned up a couple of errors that should not have caused any issues when looking at the file structure, I think they were complaints about missing DOCTYPEs).

Thank you very much for taking the time to try this on your devices and to play with the file, DNSB.

Knowing that you were only able to reproduce this on a modded device made me reflect on what changes I've been making to mine. At first I thought none, as no patches are installed on it, but then I remembered I had applied a custom English hyphenation dictionary to it. Replaced it again with a smaller file and now this book (and a couple of others which were giving the same issue) are not crashing my Kobo anymore.
