Error while parsing file: different playOrder values for navPoint/navTarget/pageTarge

Hi, I have a problem with two errors reported in the .ncx file, the error is:
Error while parsing file: different playOrder values for navPoint/navTarget/pageTarget that refer to same target
however, observing the two lines in question I do not seem to see errors, navpoint and play order have the same number, what could the problem depend on?

Are there two navpoints whose urls point to the same destination file (and same fragment if any)?

Simply regenerate the TOC. This'll fix whatever problem caused this error.

Regenerated the toc, the previous errors disappeared and these 2 appeared:

File listed in reference element in guide was not declared in OPF manifest: OEBPS/.

Referenced resource 'OEBPS/' could not be found in the EPUB.

Open content.opf and delete the <guide> entry that causes the problem:

  <guide>
    <reference type="text" title="Text" href="OEBPS/"/>
    ...
  </guide>

If you can't identify it, delete the complete <guide> section. Afterwards, you might want to right-click the cover page, the toc and the beginning of the book and add their semantics. (Use Text for the beginning of the book.)
