TOC editor minor bug

The # part of the previously created entry gets appended to the next entry upon creation if this entry points to the top of the file.

Steps to reproduce:
Create entry on top of the file,
create entry pointing inside the file,
create entry on top of the next file,
close TOC editor and reopen it.

64 bit edition of caliber on w8.1.

Works for me, I got no # part for the first entry, a # part for the second entry and no # part for the third entry.
