It doesn't matter what input file I use. Or what format I convert to or from. Every preference setting I've changed, has had no effect. The changes persist, but they just aren't used. I've upgraded Calibre from 0.9.19 or thereabouts to the latest 1.10.0. Same thing happens.
Is it possible something is wrong with my installation? Where do the preference settings come from? Is there a reason the actual program is unable to obtain the modified values in the Preferences which are definitely stored, and stay for successive program runs?
I've created a book and added HTML. Then converted to EPUB, and had <h1> tags used to create chapters. To stop this happening, I cleared the XPath expressions "/" in Common Option / Structure Detection, as web searching results noted I should. This had no effect.
I've tried converting HTML to PDF. It looks great except for the large text size. I've modified the Output Options / PDF Output options as web searching suggests. I've set the format to override to various page sizes.. to no effect. I've checked the option for page numbers. To no effect. I even started Calibre in debug mode, and the list of attribute values do not reflect my changes to the preferences. They're the defaults. pdf_page_numbers for example should not be False.

I apologise for wasting your time. This set me straight.
It turns out the conversion window was opening with the left hand side off screen. I didn't even know these options were available on a per-book basis.
Calibre is fantastic piece of work, and with a little helper script I have it generating a decent looking epub from a collection of downloaded as zipped html google documents. Now to try and tweak the PDF output to look acceptable..
