MISSING HEADINGS AFTER CONVERSION

I have successfully used Calibre to create NCX files for the first two volumes of my book by converting a perfect PRC to MOBI file.

After converting the third volume, all, except one of the chapter titles are missing in the body of the text of the resulting MOBI file, even though they are in the TOC and NCX! Any suggestions?
