Should there be any empty lines or paragraphs

In the Sigil epub document we are formatting should there be any blank lines as code or blank paragraphs as code between the end of a chapter and the start of the next one?

Are we talking individual file per chapter? IMHO No, this can cause annoying blank screens on many reader devices.
OTOH If you are doing this in a larger, many chapter file, I would create a 'chapterbreak' style (similar to a scene break) and set the treatment to taste

Code:

<p class='chapterbreak'>&nbsp;</p>
.chapterbreak {display: block; margin: 2em; text-indent: 0; }

You could use a HR ( I would still use a dedicated style so seasoning could be adjusted)

The type imply that each chapter starts at the top of a new page:
Single chapter per file - No
Multiple chapters per file with page breaks - No

Chapters follow one after the other unbroken:
Multiple chapters per file without CSS to break the page - Depends on how you want to style things - an empty paragraph to pad between the tail paragraph and new header is likely a good idea.

But better that use <div>. It's not a paragraph after all, it's just some vertical spacin. Or use margin-top/margin-bottom.
