CS5 to ePub conversion is creating repeating pages

Hi all, Been struggling with this for a while. Working with some folks who have converted a large book from CS5.5 to epub and almost everything is perfect, but in some instances we're seeing the last page repeat at the end of a chapter - sometimes once and sometimes up to 11 times, and sometimes it's blank.

Sometimes a chapter has repeating pages in one view (ie: horizontal view) if we turn it to the other view, sometimes the repeating pages are not there? One chapter (which never had repeating pages in the past) now had a weird repeating page in horizontal view... when I flipped it to be vertical view, the repeating page was no longer there?

We've looked everywhere and haven't found a solution to this and wondering if anyone here has any ideas.
