Because of a typographical error on Adobe's part, when a book is exported and distributed to Apple an error code comes up saying it doesn't like or and Apple will send the offending title back for corrections.
There is an extra space in the DTD: <!DOCTYPE html PUBLIC "-//W3C//DTD XHTML 1.1[EXTRA SPACE IS HERE]//EN" "http://www.w3.org/TR/xhtml11/DTD/xhtml11.dtd"> which causes this issue. Remove the space in any/all HTML files in your book and iBooks will find no issue with your non-breaking spaces and your discretionary hyphens. Thanks, Adobe, for once again screwing up a teeeeeeeeeeny thing that causes an entire book to have an issue every time. Let's hope there is the possibility of a patch for this..
