all toolbar items not displaying?

I'm having an issue with calibre in which all toolbar items do not display. When I first start up calibre I can see all of the toolbar items I have selected to display, but it seems like as soon as I begin using calibre to do anything, they disappear, leaving me with a reduced set.

Any ideas about the cause of this problem? Thanks in advance to know who knows the solution.

You can Customize the toolbars under Preferences.

Note that there are different main toolbars depending on whether you have an ebook device connected via USB or not. I would guess this is your problem and that icons are disappearing because you have plugged your reader in and the missing icons are not on the 'device connected' toolbar.
