iTunes Producer Error -- In need of help, please!

I created a fixed-width ePub (approved thru ePubCheck). But when uploading through iTunes Producer I get this: "ERROR ITMS-9000: META-INF/container.xml in sample-photo-book-portrait.epub must contain one and only root file reference. at Book (MZItmspBookPackage)"

My container.xml seems fine:

Honestly, I'm at a total loss here and am in desperate need of help!
