Stand Alone Viewer
I have followed instructions to setup standalone viewer in OS X, but when using "open with" the viewer option is greyed out. Any ideas?

What instructions were you following? In other words, what exactly did you do? What filetypes doesn't the open with work for, and are there some it works for?

I used the command line tools on OS X first by going to Calibre->Preferences->Advanced->Miscellaneous. Then went to finder and tried to get mobi files to open with Calibre Viewer. But it does not give me the choice.
