Last time I talked to an American librarian, the cost was $0.08-0.12 for each library loan plus Amazon's bandwidth fees which depends on the size of the ebook file.

I finally got around to asking my librarian about Kindle library loans..

My library, which uses Overdrive, does not pay anything extra on a per loan bases for a Kindle checkout. There is a single upfront cost per ebook (regardless of ebook format), and then there is no extra charge per loan.
