There should be two koreader.app files in the zip archive you download. The main one (larger file size) goes in the applications folder. There should also be a simple launcher version (smaller file size) that goes in ./system/bin

The simple launcher version allows you to press and hold on books from the pocketbook library and choose to open with koreader; it will also allow koreader to launch the last book read on startup.

Opening books directly through koreader rather than via the library will prevent them opening automatically on startup.

Check that Koreader is set as default for ebooks...
Main menu of KOreader:
cogwheel -> device -> link file-extensions : Enable all

KOReader comes with platform-specific setups too, so your patches may be useful there.
You can just submit a PR in koreader/koreader on Github and wait for feedback

Mhh sure. I will look into it. Maybe I have some time over the winter holidays.
