I use a book pillow myself and find it very handy, not only for bed but also when reading a big hardcover in a chair or at the table.

I use a Micro-Puff, which is flexible and can be adapted to your body or any surface. I usually use it for my bulkier, heavier e-readers, both on the sofa and in bed.

If I read in bed on my iPad Pro, I have no problem with it as it has an Apple Keyboard which is also a stand.

I have a ValarivGame Pillow (HERE). While I primarily use it for gaming, I also use it for reading, especially larger books that I do not want to hold. It looks quite goofy but my shoulder pain from hours of gaming or reading is all but gone since I started using it.

I also have an adjustable bed, so if I read in bed, I am doing so at a sitting angle.

I've got one like your second picture with the bendy stem. Because my bedroom is cold I also have a clip on page turner so I can keep my hands under the covers.
It's a reading revolution.

I’ve got a headboard bed for this exact reason. Back in the age of paper books I was able to hold paperbacks quite comfortably and hardcovers by resting them on my torso. As with an ereader the trick is to make sure your elbows are well supported on the matress.

Thinking back to how heavy some of the bigger hardcover books were and how I had to put then on the matress and lie on the side to read them, I find holding and reading on ereaders and even my tablet in bed, very easy.

I read in bed with my head propped up with a couple of pillows. Hold my Kindle Paperwhite with my hand.

This. I never read in bed. I go to bed to sleep. I've had insomnia problems in the past and it's best to reserve the bed for sleeping. If I can't sleep, I get up. My wife likes to fall asleep watching TV which I've gotten used to. It's background noise which helps if my tinnitus is particularly bad that day.

I read Wikipedia articles in bed sometimes, on my phone. That doesn't spoil my sleep (I don't read or sleep in complete darkness, and I've never suffered from insomnia). I don't read books in bed, as I don't like to do that while sleepy. And I only go to bed when sufficiently sleepy. If I don't feel sleepy, I'll stay up until I do.

IMO, 6" e-readers were made for reading in bed because they're easy to hold without some Rube Goldberg contraption.

The Pocketbook Verse Basic doesn't boast great specs but its 6" display is perfectly readable, it offers the terrific PB UI, it is inexpensive and it is the only Linux-based e-reader with an sd-card.

That's an unbeatable combination for reading in bed.

At least for me I'll probably use this contraption as a smartphone holder while exercising (calisthenics) to film myself and review I'm doing the movement correctly, with good form. The extra flexibility an articulating arm provides would make setting this up easier. At first I was looking at getting one of those little tripods for that, but getting something that I would get more use out of makes sense to me. As long as I won't find moving it back and forth 3 times a week too annoying, of course.

I find the Kobo Libra 2 with it's 7" screen very easy to hold in bed with one hand.

It's not just the size of the screen, but also the weight of the Reader. I know there have been some 6" Readers that were rather heavy in comparison to the Libra 2,

I use one of those pillows (https://www.amazon.it/-/en/Purovi-Er.../dp/B011INGXFS / https://www.amazon.it/dp/B06XHT9GRV/ ), in its vertical stance, so I can bend the legs and lean the e-reader, or the book, on the thighs/knees.
But lately reading almost on a chair, which swings and can put the foots on a pouf.
