It gets the chapter title from the code near the top of the HTML file.

Slab serif are not good for reading a book.

Replacing the entire CSS is not good. You should learn to edit it.

The simplest most basic epub I could create, and converted to kepub via calibre

It picks up actual chapters ok, but the front matter is a mess -- like, the copyright "chapter" has 4 pages that includes dedication etc...

I messed with it for a bit then gave up on it. The h-tags didn't seem to work either, as I use them to create the "embedded" toc for the kindle, eg. to include "Also by the Author"

Oh I wonder if it's from the content.opf, taken from the <guide>...

There must be something mighty peculiar in the way you create your epubs. I've converted literally thousands of epubs to both azw3 and kepub (after editing the css/html), and nothing will get messed up if the book is reasonably well formatted.

What happens if you use the KoboTouch driver to convert to KePub as you send the ePub to your Kobo? It sounds like what you are doing is an inplace conversion to KePub which is not what you want to do.

More than likely

I don't want to use calibre though, and Kindle Previewer only does .mobi really. I can open those files and it's exactly what I've created in the epub, no code added or changed.

No idea what that is so not using it

Used calibre, some online thing, and just renaming the epub. None of them did the "chapter title" right for the front matter.

It's not that important--which is why I asked the original question. If I can gut an epub of all pretty style, layout, and fonts, basically just dump a simple text onto the device without it breaking, that'd keep me happy.

It's only for the boy he won't care. I, on the other hand, like a bit of polish. But just a little, mind...

Go to the following link and download the Star Trek ePub. I made it from the original word document. So I know it's well made.

Also download the attached .azw3 file and load that on your Kindle. It was conversion using calibre. No editing was done to it. You should see the chapter titles correctly.

As for the front matter, that rarely has a chapter title. The front matter in this case has no chapter title.

If I remember correctly, that is from the <title>Text Goes Here</title> in the <head></head> section at the start of the xhtml file.

Personally, I just use Sigil's Preview and the optional BibiReader, EpubJSReader and ReadiumReader plugins to get an idea of what the ePub is going to look like.
