Add support for calibre-web-automated hardcover-id and hardcover-slug identifiers (fixes #92)

Update README with new matching flow

Note: the existing hardcover identifier is still used, but may stop being set as an identifier, in favour of hardcover-slug (as otherwise you've got two duplicate identifiers)
