I've been trying to investigate the issue where using the "Update ToC" feature sometimes causes a "unique constraint failed" error, but so far I have not been able to reproduce it. @ownedbycats gave me some pointers about the EpubMerge and FanFicFare plugins, but even when experimenting with them a bit I've been unsuccessful, so clearly I'm missing something.
I'm assuming that the basic reproduction steps required are these:

Did you have Do not treat replacements as new books selected on the 2nd tab of the KoboTouch driver configuration dialog when testing? If not, the version 2 would replace the version 1 including the ToC.

Yes I do have that selected, I'm assuming that's what people who experience the issue use as well. Otherwise as you say everything would be replaced and updating the ToC wouldn't do anything.

Does the ToC actually need updating? Normally I only update ToC when the fic updates. Adding an additional ToC entry should emulate that.

I was finally able to reproduce the issue by enabling the "Flatten table of contents" option in the EpubMerge plugin. So I should be able to investigate this now.

Hello. I have used the plugin to change the covers of books on a Kobo Libra Colour, but although the image dimensions are correct in the Calibre library, when I hover the mouse cursor over the book covers on the device, the resolution displayed is much lower. For example, a cover that I have at 780x1200 pixels is approximately 500x800 in the book. I have tried deleting the book covers from the Kobo and sending the covers using the plugin, but there is no way to maintain the resolution. Any suggestions? Thank you.

You might want to remember that your Kobo stores the covers with 3 different resolutions plus the cover image stored in the ebook. Since your Libra Colour does not have a mouse, I'm assuming that you are looking at the book covers using calibre.
If I look at the .kobo-images directory on my KLC, each book has 3 images which for me show as shown in the image below (I renamed the files to *.jpg). These files are generated from the cover image during the import process. When I look at the details in device view of a book, I see the resolution corresponding to the N3_LIBRARY_FULL.parsed image. |

Ah, thank you very much, DNSB. I see that I have those files in folders (with numbers instead of book titles) and that by adding the .jpg extension I can check what you say, and indeed everything seems to be correct. I did check the resolution in Calibre, in the Device section... and I don't know why it's reduced there, but I'm not worried about it anymore. Thanks again.

Calibre shows the cover from inside the ebook. Changing the cover on the device doesn't affect what's in the book.

I may be mistaken, but if I used the plugin to delete the book covers from the device and then again to send the covers from my library to the books on the device... shouldn't the covers inside the books have been changed as well as those in the directories referenced by DNSB? All the books I put on the Kobo had covers measuring at least 800 x 1200 pixels, and in Calibre's preview of the books on the device, those covers are displayed as just over 300 x 500; so I assumed that it was showing one of the files in that directories and not the covers inside the books. Sorry if I'm mixing things up, but I'm not really sure how it works, to be honest.

For example, the following sequence does not change the cover in the book.
The cover inside the book is normally updated to the cover in calibre's library when you send the book to the device. If you want the cover inside the book on the device to change then you must resend the book after updating the cover in the calibre library. |

Just before sending my books to Kobo, I replaced the low-resolution covers with higher-resolution ones and clicked on “embed metadata”; that's why I don't understand the low-resolution images displayed in Calibre; I must have done something wrong then.
