How To Generate a New Table of Contents

When writing a book, much of it gets edited down, including whole chapters getting deleted. So ...

How do I delete the existing TOC and just generate a new one (from H1 chapter headings).

It (the NCX) will just be replaced if you use the TOC tool:
select all existing entries: Delete (the recycle icon)
then Generate a new TOC from headers or whatever)
