After spending 12 hours struggling with this same thing (I'm using Pages --> epub -->Calibre --> .mobi), I figured out why I was having the same issue: My chapter headings were not formatted with the default chapter format; they were using something I'd created (don't know why) called "Chapter 2). The export would not register these as valid chapter/page breaks. Make sure you're using a valid chapter formatting.
