I can't figure out why someone would smile at receiving a human arm and then toss the entire mess in the garbage.

Okay, I will try it. But it is very lateral and it is only logical if it makes sense from a medical point of view.

The man in the house has a twin brother, this brother has bone cancer in the arm and needs an arm transplantation.
The man in the house went to the hospital a few days or so ago and the doctors took off his arm. The wound healed and now he's home again.
Now the brother is in a hospital, the doctors took off his sick arm and transplanted the arm of the man in the house.
The arm in the parcel is the brother's arm, it means that the operation did work out well.

Yes, I admit, it sounds abstruse, don't kill me. Hopefully it gives someone else an idea into the right direction.
