Its definitely a problem in the rtf output plugin.

I don't want to look too stupid here, but would that be in:src/calibre/ebooks/rtf/rtfml.py ? I know that's the file user_none modified to correct the epub->rtf conversion problem for me. I even wonder if that change caused this problem, if italic conversion worked at one time, and later stopped. I'm not sure how often rtf conversion source was being changed.

Yes basically the files involved in rtf output are all in the ebooks/rtf directory. The entry point is rtf/output.py and the heavy lifting is done by rtfml
