Different people have different preferred ways. I use Calibre's network share ability (preferences/Share over the net) and then use Marvin's OPDS feature to import. At one time, it was possible to connect to your computer and use the Calibre Marvin plugin, but that option hasn't work for a while.

You can do a "Save to Disk" from Calibre, and then drag and drop the books from Windows Explorer onto the Marvin application in iTunes. i'm not aware of any method for exporting annotations from Calibre, though. To be honest, I wasn't even aware that Calibre stored annotations!

I'm afraid I don't understand the first part of your question. Yes, you transfer the files to the iPad using a USB connection.

What if you wanted to copy 100 files to the iPad? Can you do all 100 in one go from Dropbox?

If you have the Dropbox app on your iPad, Marvin will actually link to your Dropbox account, catalog all the EPUB files you have on it, and then allow you to choose which ones to download. It's very handy.
