Get a new battery. Not much else to do if your battery isn't holding enough charge anymore.

You can try a few other things first.

1. Take the battery out, make sure contacts are clean and put it back in.
2. Turn the phone off and on.
3. Try another charging cord.

Other than that, you will need a new battery. If it is under warranty, take it back.

removable battery only on original Galaxy Nexus. try replacements from eBay.
