My workflow is:
  1. Edit file and save changes
  2. Close editor
  3. Connect device via USB
  4. Send file to device
  5. Eject device
  6. View book on Kindle
  7. Remove book from Kindle using Kindle UI
  8. Goto 1
After a couple of cycles of this, I hit the described bug.

What I am doing is checking that my style changes work correctly, and only the actual reader on the Kindle seems to be 100% accurate with what I will see.

I'm sure this is not a Calibre error, but I would like to know if other users have seen similar loss of their preferred theme. Until I found the workaround, I thought I was completely messing up the CSS, but couldn't figure out what it could be.
This happens to me occasionally, sometimes even with books that I've purchased from Amazon and downloaded directly from them. I don't think it's a problem with Calibre, or the way you have formatted the book, I think it's just a bug with the Kindle.
