I have that same issue happen, occasionally, with blank pages on some EPUB to PDF conversions, and I found that sometimes the only change needed in the settings was in Look and Feel -> Layout tab -> Remove spacing between paragraphs, whereas in other cases some CSS modifications were needed in Calibre Editor.

Using my current settings, including the one I mentioned, I got no blank pages with that epub, in the mentioned locations. I think it should be better to scramble it before posting on MR.
