The error "E_LIC_ALREADY_FULFILLED_BY_ANOTHER_USER" means that the ACSM file you're trying to add is already linked to another Adobe account.
This either means that you bought a new book but opened the file in ADE or on your eReader first before importing it into Calibre; or you re-downloaded an old ACSM for a past purchase and are trying to download it with a different Adobe account.
If you get that error message, there's only two ways to fix that: Either find the old account that the credentials are linked to (if that account is deleted, is a lost anonymous authorization or you don't have the password and can't reset it then that's not possible), or you try to contact the shop where the file is from and ask them to reset it and send you a new unused ACSM file. They might not be willing to do that, though, as they'll have to pay for that.
