You can install extensions/enhancements. It's called plug-ins.

If you did fork Calibre just for one column name, I would think nobody but yourself would use that version. Now if you forked it and kept up with the official changes, then maybe others would use it. But you would have to change the code to check for a new version of your fork instead of a new official version and you would have to compile for Windows, OSX, and Linux.
