OSError: [Errno 1] Operation not permitted: '/Volumes/Kindle/documents/Alexander, Lloyd'
The errors suggests the device has been mounted read-only or some other permissions have changed to prevent calibre from creating a new directory. There have been plenty of reports of MACs mounting ereaders read-only, so that is probably it.
