What format is the SD card formatted with? I would suggest downloading the SD Association's formatter available from SD Memory Card Formatter and reformat the card either in the Kobo or externally. You could check the current format and see if the card is formatted as FAT32 or exFAT -- see the attached properties display under Windows. The SD Association formatter will format as FAT32 which is what your Kobo is expecting.

Another possibility is power cycling your Kobo with the uSD card installed if it still is not seen after using the SD Association formatter though that was an issue more with the older Touch ereaders as far as I recall.
