Side-loading of books does not enter into the situation. In fact, the books do not even have to be present on the (grayscale) Kindles.

Yes, my main thought as well:
(1) create the collections somewhere else, to appear as Cloud Collections;
(2) use that "somewhere else" to populate the collections with books following a certain folder structure (similar to librariansync)
(3) trigger the Kindle sync function and have the collections already populated.
My feeling is that (1) and (2) can be done programmatically, even if this is not quite easy.
