@KyBunnies - That's odd, maybe another extension or your theme is interfering. My settings are (the other two tabs have the topmost option selected):

But, I do have the previously mentioned MR profile option set to All Links, don't recall ever changing it. And it seems to be the default setting anyway. And I also middle click a lot, my netbook keeps a three legged pet mouse in its pouch

Try creating separate profile. You need to put firefox -p in Start Menu->Run to do that, then install the Open Link in New Tab extension in the new profile. Its quite useful to have an empty profile available for testing, investigating and evaluating extensions.

Another thing to try is disable all other extensions and themes. I found a conflict by doing that only yesterday.

@eschwartz - I stopped using tpm, mainly because of its performance penalty, also I'm not sure it adds much value these days. I use the Tabloc extension to lock tabs - and pin tabs is really useful.

And I use the Open Bookmarks in New Tab extension.
