It's a recurring Kobo server issue that causes the duplicate shelves (not firmware or calibre).
The ONLY reason that calibre won't work with 3.12 currently (perhaps excepting the fact that a plugin doesn't seem to be removing duplicate shelves properly) is because the "supported" firmware version number hasn't been updated in calibre yet. Davidfor tests calibre it all the way thru the firmware beta's and if there was an actual issue he'd have posted it.
As PeterT said, in order to get calibre 2.12 to work with Kobo firmware 3.12 "you just need to select the option Attempt to support newer firmware in the Customize option for the driver.". Trust me, Peter knows what he's talking about.
