It means that as soon as an e-mail hits the mail server it is sent immediately(*) to your phone.
The other option is pull e-mail where the e-mail client polls the mail server at regular intervals to check to see if there are new messages, and downloads the lot at that time.
(* for random values of immediate, given that things can go astray in the ether and so on)
