Somehow, I must have been wrong about the 60 days. I read that from a post that I had gone back to from the end of April. The credit must have been good for 30 days, as I initially had in mind, instead of 60. I am sorry if I led anyone astray; if it is any comfort, I led myself astray! To add to my frustration, Audible kindly sent me an email a few days ago reminding me to use my credit, but I didn't open the email, thinking that it was an earlier credit that I had already used up!
Of course, I am thankful for the credit that I got for free that I was able to use.

You might want to dig into this a little further. Reading over at Goodreads I see several people who had both the $10 from Buy-4-Get-$10 and the Avis deal complained that they still had some Avis money left but it got wiped out when the end of the month (when the 4-10 money expired) rolled around. If you think this happened to you call Audible's customer support and you'll likely get some love.
