AFAIK, he didn't die of cancer, but of heart failure. And in the later Granada episodes, he got bloated because of the drugs he took for his manic depression.
And sad as that is, his own issues may have made him a better Holmes (he seems to have thought so, and calling Holmes a manic depressive is probably not far off the mark...)
