Thats fine. You're on 3.0.3 and nothing changed the OTA version at all which is expected.
Updates are made into a separate directory and only copied into the main firmware area when all the files have passed the hash check and all the patches have applied successfully.

Huh? That doesn't make any sense. When it is in USB drive mount, that partition is unmounted for the Kindle OS which is why it has USB drive mode as FAT isn't suitable for multiple devices to access concurrently. USB drive mode just exports the raw partition from flash to the computer so there isn't any extra staging area for changes.
You should scan the Kindle when plugged in with USB drive mode for errors or a corrupted filesystem.
