When it's in the case, that's how I hold it, though. Doubling the front cover around to the back, it provides enough thickness to hold it that way, without my frog cramping. (I'm loving the frog thing.  )
In bed, naked, or in the case, I usually hold it in ways that I'm afraid are... weird.
First, I rest the bottom on a pillow.
Then, when it's in the case, I put:
Middle and pinkie fingers around the back.
Index finger in the space between the covers.
Ring finger over the page turn button.
I click the page button kinda backwards - with the dorsal side of my ring finger, between my first and second knuckles.
Without the case (which is how I got started doing that - see cramping frog), I put all fingers around the back, except my middle finger, which then does the clicking. Don't ask me why. I dunno. I think it's because it's stronger, and without the thickness, I need more leverage...?
Am I the only person who holds it anything like that?

I do the resting on the pillow bit, but the rest seems AWFULLY convoluted to me! it'll be interesting to see how this whole frog thing sorts out!
