TOC Problem

I have a html file that I am trying to convert to epub. The html is setup as:
<h1>PART 1</h1>
<h2>Chapter 1</h2>
<h2>Chapter 2</h2>
...
<h2>Chapter 17</h2>
<h1>PART 2</h1>
<h2>Chapter 18</h2>
<h2>Chapter 19</h2>
and so on

For some reason the TOC comes out with only headings 1 through 17, as if they were heading 1s. It did not pick up any of the h1s nor any of the h2s past chapter 17. It stops at PART 2. This book has 4 parts and 50 chapters.

I have Calibre set to force use of TOC and Level 1 = //h:h1, Level 2 = //h:h2. Why does it not recognize all the headers? I've gone through the source html and all the headers follow the h1 and h2 shown above.
