Tried with two books - no luck. Even if I put device in airplane mode before sending the book - after device is disconnected it has already an icon as on "Your Clippings" txt file.

As another test - one book was converted with Calibre (ePub -> AZW3) and after sending to Kindle with enabled airplane mode - the cover was there. I think I missed something while I was converting previous two books using Kindle Previewer.
