Python, with many 3rd party libraries. You have a big job ahead. There's no substitute for digging into the code, although Kovid can probably give you pointers on where to start.

First read the conversion section of the user manual to get an overview of the conversion system. Then look at a plumber.py to see the actual code that drives the conversion pipeline, then you will come up with ideas for how to make this happen, the first few of which I am likely to shoot down
