Maybe I missed something. Like why calibre is installed to D:\Applications and not something like D:\Applications\Calibre2? Or C:\Program Files\Calibre2?

That's not going to do anything. Setting that environment variable will have no effect on the qt.qpa.plugin not being found.

@straygrunt you could try safe mode and see if calibre works. But before doing that, uninstall the calinre you have now and reinstall it to the default directory of c:\program files\calibre2.

There's typically no problem with installing to a different location (other than possible permissions issues in the new location), but when calibre's binaries are located in a folder called D:\Applications, then I worry that the contents of what is typically the Calibre2 folder have been dumped into a folder that has other other binaries that could be using other versions of Qt.

Actually, you caught something I missed. When I used the FAQ to uninstall/reinstall, I think I pointed it to D:/applications thinking it would create a folder there called Calibre. I cleaned out there root of that folder and reinstalled again. Looks like it's working. I really appreciate the help on this.
