I've tested this only on various Kindles in KF8 format. I'm not sure how things will render on various epub readers.

Why isn't the TOC complete?

Ebooks are not paperbooks. So we are not using classical TOC-pages inside the book, when we have a much better tool. The everywhere and at all times by TOC-key requestable TOC.
