element "img" not allowed in this context

I am getting the following error while trying to upload an ibook to itunes (with itunes producer) converted to .epub with calibre :
ERROR ITMS-9000: "element "img" from namespace "http://www.w3.org/1999/xhtml" not allowed in this context" at Book (MZItmspBookPackage)
ERROR ITMS-9000: "required attributes missing" at Book (MZItmspBookPackage)
What could be the problem? The cover art?

I haven't done this but I recall this error means you have to have the alt="" attribute within the img tag.

make sure the class attribute on the element has a simple value starting with an alphabet and contaiing only alphabets, numbers, spaces.

The fix for this for me was putting the <img tags inside a div which we called image container.
