In my test, I only see the title of h1 is shown, will it be possible to show titles of h2 and h3 as well ?

No only top level titles are going to be shown. It's too much noise otherwise.

Understood. However, the authors sometimes use the top-level to group chapters as Part 1, part 2... in the book. At least, supporting h2 would be great. Thank you for the considerations.

I'm afraid if you want that level of customization you will need to write your own code for it. Export the annotations in json format instead and post process using your own code, however you like.
