How do I customize this?
I have 2 questions.

1. How do I customize the toolbar which is present when a book is opened.

That appears to be the ebook-viewer which has its OWN preferences (on the toolbar)
The main Calibre GUI preferences do not affect the INSIDE toolbar of the viewer

The viewer toolbars cannot be customized, only their position changed.

And there is no way to make the scrollbar per chapter, I'm afraid. It's hard to do because chapters are not well defined, they can start at any arbitrary point in the HTML. That greatly increases the amount of work needed to implement per chapter scrolling/time left indicators.

If your book has a NCX TOC, the TOC can be used.
Unfortunately, not all book publishers consider a full TOC necessary and onlyinclude what is necessary to get past the gatekeeper at various publishing portals
